This option is ideal if you have many students taking the MLP exam. Down load the Excel Registration Form, then upload the completed form and submit it here.

Online Registration
This option works well if you have fewer than 10 students taking the MLP exam. You have to submit this form for each student.
Instructions: Please complete all contact information. Include students' first and last names. For each testing area, select the student's level. If a student is not participating in a particular area, select "not taking this portion". Students may participate in up to three options per test.. Under State Fee, enter $5 for each student, per level they are taking (up to 5 areas plus options). Enter your chapter's fee in the next column. All fees will be added automatically. Make one check for the total amount to your chapter.
